Above prices are based on quantities for a single card design.

Multiple card designs will be billed at the total card quantity plus a $ 900.00 charge per additional card design if this is lower providing, the additional design(s) are printed at the same time.

Reprints will be billed at the original quantity price plus $ 900.00. A minimum of 2,000 forms will be required.

To personalize scan cards with employee name, company logo or other employee information, add $ .50 per card

Scanner Exchange Program ? A 24 hour turn-a-round exchange or refurbish program is available (call for details and pricing).

On site training is available for $ 500.00 per day plus travel and expenses.***
