HOW TO FILL OUT THE PLEDGE FORMS & MAKE CORRECTIONS
It is very important to instruct employees how to properly fill out the pledge form and make corrections.

The SDSI “United Way the Easy Way” system uses laser imagery to accurately retrieve data from the pledge forms. Here are the guidelines for filling out the cards and making corrections.
1. No. 2 pencil is preferred and works best.
2. Black or dark blue ballpoint pen will work.
3. No red or green pen. These are drop out colors and will not read.
4. No felt tip pens.
5. No liquid white out.
6. Dry correction tape only. For best results, use your finger to press the tape flat after applying to pledge form.
7. Do not mark outside the bubbles.
8. You do not have to fill out the back side.
9. If you do not wish to contribute, Sign and return card only.
